People who do not have health insurance or choose not to use it generally have federal rights to expected-cost information before scheduled care.

Your right to an estimate
Under the federal No Surprises Act, healthcare providers generally must give uninsured or self-pay individuals a Good Faith Estimate of expected charges when care is scheduled at least three business days in advance or when an estimate is requested. Emergency care is not covered by this advance-estimate process.
What the estimate means
A Good Faith Estimate is not a bill or a guarantee of the final amount. It should identify the reasonably expected charges known at the time for the items or services described. Unanticipated needs, changes in the planned services, or services from another provider or facility may affect the final cost and may require a separate estimate.
Request and keep a written copy
- Tell the practice if you do not have insurance or do not plan to use insurance for the requested care.
- Ask for a Good Faith Estimate in writing before scheduling or before receiving the item or service when applicable.
- Review the services and expected charges listed on the estimate and ask questions about anything you do not understand.
- Keep the estimate and compare it with the bill you receive.
If a bill is substantially higher
CMS states that a bill may qualify for the federal patient-provider dispute resolution process when a provider or facility charges at least $400 more than that provider’s Good Faith Estimate. A dispute generally must be started within 120 days of the initial bill. Eligibility, fees, deadlines, and procedures can change, so use the current federal instructions.
